Potomac Home Health Care, a non-profit joint venture of Sibley Memorial Hospital and Suburban hospital, is a full service, medicare-certified home health agency providing a comprehensive range of home care services in Maryland and Washington, D.C.  Working within a team approach, we make every attempt to recognize and meet the needs of the homebound patient. 

Our program stresses personalized care, tailored to the individual patient's needs.  We work closely with the physician, the patient and family members in planning, reviewing progress and making adjustments to the plan of care.  This approach results in a better treatment plan and greater understanding and support for the patient and family.  Our program has the unique capability of offering the resources of two full service hospitals. 

To be eligible for service, a patient must:

• Be confined to the home;
• Require skilled nursing or therapeutic services on an intermittent basis; 
• Have a home health plan established by their physician.

Potomac Home Support Inc, a self pay agency,  is a sister company to Potomac Home Health Care and a Service of Sibley Hospital and Suburban Hospital.  We are a non-profit coporation providing private duty services by way of Homemaker/Housekeeper, sitters/companions, live-in and certified Home Care Aides, transportation assistance, in the M.C., P.G., and Washington D.C. area.  We are available to provide your loved ones with the personal attention and care they need at home or in the hospital.  Licensed nurses supervise our trained and experienced field staff.  Personnel are bonded and insured.  For more information on Potomac Home Support click on the PHS link on the left, you will be redirected to PHS's website.
