Skilled Nursing

The registered nurse performs a comprehensive nursing assessment, develops a plan of care and coordinates care with the physician and other team members. The nurse performs nursing procedures and assists patients and caregivers to become independent in care by instructing them in disease process, medications, and symptom management.

Infusion Therapy

Our patient-focused team of professionals provide comprehensive infusion services including hydration, antibiotics, chemotherapy, total parental nutrition, pain management, blood products, catheter care and enteral nutrition. Our infusion nurses promote and environment conducive to the successful completion of therapy and the sage delivery of care.

Mental Health

A multi-discipline team of credentialed mental health professionals provides psychiatric care to patients with a primary psychiatric diagnosis or a medical condition complicated by a pre-existing psychiatric disorder. This specialized service offers a multitude of interventions to ensure the continuum of care between an acute inpatient milieu, day program and/or outpatient services. This approach facilitates the patient's return to his/her highest level of functioning.

Medical Social Work

Licensed clinical social workers (LCSW & MSW) participate in the development and implementation of the plan of care by assessing social factors which impede recovery and coordinate use of community resources and special support groups. Social workers also counsel patient or family members if there is a need related to loss of health and function and/or a need for psychiatric services.

Physical Therapy

The licensed physical therapist assesses the patient's functional level in the home by evaluating the patient's strength, range of motion, endurance, coordination, balance, pain, posture and mobility. The physical therapist develops and carries out the rehabilitation program in order to maximize total function.

Occupational Therapy

The licensed occupational therapist assesses the patient's ability to perform activities of daily living and fine motor functions, the need for adaptive equipment in the home, and the patient's cognitive and perceptual function. The occupational therapist develops a specific program geared to maximizing the patient's safety and independence.
